What are Service Pathways?
Service pathways are:
- Powerful roadmaps of care;
- Tools to clarify service delivery/interventions to youth and service providers;
- Especially important in integrated service delivery models of care; and
- Evidence-based tools to reduce clinical variation and improve outcomes.
Types of Service Pathways
- Youth Wellness Teams Pathway (e.g., mental health, substance use, primary care, crisis support)
- Community and Social Support Services Pathway (e.g., employment/education, peer support, navigation, housing, outreach, family supports)
- Skills and Well-Being Activities Pathway (e.g., recreational)
- Virtual Service Pathway (e.g., all the above!)
