- To guide and support care of children and youth between the ages of 12-25 presenting to the hub with mental health/substance use/ health concerns;
- To ensure seamless transition to higher intensity services and follow-up services; and
To enhance care for young people and their families in the areas of: quality, safety, integration, and efficiency.
