What are Service Pathways?


Service pathways are:
  • Powerful roadmaps of care;
  • Tools to clarify service delivery/interventions to youth and service providers;
  • Especially important in integrated service delivery models of care; and 
  • Evidence-based tools to reduce clinical variation and improve outcomes.

Types of Service Pathways

  • Drop-In Pathway (e.g., recreational)
  • Non Clinical Pathway (e.g., employment/education, peer support, navigation, housing, outreach, family supports)
  • Clinical Pathway (e.g., mental health, substance use, primary care, crisis support)
