What are Service Pathways?
Service pathways are:
- Powerful roadmaps of care;
- Tools to clarify service delivery/interventions to youth and service providers;
- Especially important in integrated service delivery models of care; and
- Evidence-based tools to reduce clinical variation and improve outcomes.
Types of Service Pathways
- Drop-In Pathway (e.g., recreational)
- Non Clinical Pathway (e.g., employment/education, peer support, navigation, housing, outreach, family supports)
- Clinical Pathway (e.g., mental health, substance use, primary care, crisis support)
