Clinical Service Pathways

Purpose of Clinical Service Pathways:

  • To guide and support care of children and youth between the ages of 12-25 presenting to the hub with mental health/substance use/ health concerns;
  • To ensure seamless transition to higher intensity services and follow-up services; and
  • To enhance care for young people and their families in the areas of: quality, safety, integration, and efficiency.

Essential Components to Clinical Pathways:

  • Timeline
  • Categories of care, activities, or interventions
  • Standardized assessment points

What to do, when to do it, by whom, and where

Benefits of a Clinical Pathway:

  • Provides youth and their families clear expectations of care
  • Provides a means of measuring client progress
  • Promotes teamwork and integration on a multi-disciplinary team
  • Improves client outcomes
  • Improves utilization of resources
