YWHO Service Pathways
What are Service Pathways?
YWHO has developed integrated service pathways that are consistent across sites so that youth can access the care they need wherever they are in Ontario. These pathways are also adaptable in each community to ensure that each hub remains culturally grounded in the community they serve, meeting the wholistic needs of local youth and promotion of their overall well-being.
Local YWHO networks offer in-person and virtual services through a continuum of care considering a range of youth needs and an individualized treatment approach across all services. This model is evidence-based and informed by youth and families.
Service pathways are:
- Powerful roadmaps of care;
- Tools to clarify service delivery/interventions to youth and service providers;
- Especially important in integrated service delivery models of care; and
- Evidence-based tools to reduce clinical variation and improve outcomes.
Types of Service Pathways
- Clinical Service Pathway
Clinical Services include intake, screening and engagement, mental health and substance use counselling and treatment, primary care, peer support and care navigation services. Services and interventions offered are evidence-based or evidence-generating and are guided by the YWHO Integrated Continuum of Care Model.
- Community and Social Support Services Pathway
Community & Social Support Services meet the wholistic needs of youth and promote their overall well-being. These services include, but are not limited to outreach, family supports, education, employment and training, housing and financial support services.
- Skills and Well-Being Activities Pathway
Skills & Well-being Activities offer drop-in and scheduled, structured or unstructured activities meeting youths’ cultural, social connection, engagement & leadership, learning and leisure interests and needs. Activities span the range of all physical and mental health, substance use, education/training, and cultural, 2SLGBTQ+, and other specific needs. Youth/families are not required to register for these services at the hub, but this stream may serve as a pathway to other supports (e.g., Clinical Service Pathway, Community and Social Support Services), if needed.
