Rationale Cheat Sheet

Purpose: Why are we collecting this information?	We are collecting information from all youth to find out who we serve, and what unique needs our youth have. We use this information to identify client needs (for example: interpretation supports, accessibility, etc.). We will use this information to better understand and respond to youth experiences and outcomes.
Voluntary: Access to service	This information is important so that we can provide you with the services that best meet your needs. However, you can still access our services regardless of the information you provide, or choose not to provide. Your responses will not affect your care.
Confidentiality: Who will see this information?	This information will be accessible only to the team working with you at the hub. When used in reporting and/or research, information will be combined with data from all other youth and no one will be able to identify any individual.
